Community Living Support
Community Living and Support is an individualized service that enables the waiver beneficiary to live successfully in their own home or the home of their family or natural support, allowing them to be an active member of their community. A paraprofessional assists the beneficiary in learning new skills, and/or supporting them in individualized activities that align with the beneficiary’s preferences.
Community Networking
Community Networking services provide individualized day activities that help waiver beneficiaries have meaningful days in an integrated community setting. If an activity requires a fee to participate, Community Networking can refer and link beneficiaries with resources to pay for activities. Community Networking activities take place outside of the beneficiary’s primary private residence, residential living arrangement, or service provider’s home and are intended to offer the beneficiary the opportunity to develop meaningful community relationships with non-disabled individuals.
Respite Services
Respite services provide periodic or scheduled support and relief to primary caregivers from the responsibility and stress of caring for waiver beneficiaries.
Supported Employment (SE)
Supported Employment (SE) helps waiver beneficiaries obtain competitive, community-based work and provides ongoing support to ensure long-term success. SE assists beneficiaries with finding jobs that pair fair wages in a variety of settings, based on personal skills and preferences.

Silver Lining Care Management’s Direct Support Professionals (DSPs) work one-one-one with beneficiaries to complete applications, prepare for interviews, and develop job readiness skills. Once the beneficiary is hired, the DSP provides on-the-job support for the beneficiary. This promotes success and satisfaction for both the beneficiary and their employer. Support continues for as long as needed to ensure lasting stability in the workplace.
